Treating Pneumothorax with surgery: Preventing recurrence may be more effective and less traumatic than you imagine!

Pneumothorax (commonly known as a "collapsed lung") is a condition where air accumulates in the pulmonary pleura and then compresses the lung. The human body relies on the chest wall and diaphragm movement for lung's expansion and contraction in breathing. Once pneumothorax occurs, the body cannot directly drive lung movement, reducing the efficiency of the lung function and leading to breathing difficulty and chest pain. If not treated properly, there is a chance to increase the risk of recurrence.

Dr SIHOE Dart-loon Alan, Honorary Consultant of Cardio-thoracic Surgery at Gleneagles Hospital Hong Kong, states that traditionally pneumothorax is recommended to be treated with air evacuation followed by pleural symphysis, an assisting open-chest surgery. The surgery involves injecting chemical agents or placing irritants to prevent the recurrence of pneumothorax. In recent years, patients may choose the alternative option to undergo a minimally invasive approach — single-port thoracoscopic surgery to prevent recurrence. After air evacuation, the doctor will perform the surgery directly through the drainage tube incision in the chest cavity, without another incision, effectively reducing surgical trauma and shortening recovery time.

To further prevent recurrence, doctors may also incorporate an Enhanced Recovery After Surgery (ERAS) programme to help patients regain their abilities as soon as possible. For instance, prescribing pain medication or anaesthesia before the surgery to reduce the pain response of the nervous system and using electronic chest drainage to maintain stable negative pressure post-surgery to assist lung drainage and healing. Patients only need to avoid strenuous exercise or air travel for one month after the surgery to avoid affecting lung adhesion, after which they can resume normal life.
